In EverQuest, the main boss in The Sleeper's Tomb was Kerafyrm. You can awaken the Sleeper on a server only once. Originally intended to be unkillable. The dragon first awakened in July 2001.


"On November 15, 2003, on the Rallos Zek PvP server, the three top guilds (Ascending Dawn, Wudan, and Magus Imperialis Magicus) assembled over 180 players, along with an assortment of other non-PK players with Sleeper's keys, with the intent to kill The Sleeper. This was in response to an attempt to wake The Sleeper by an Iksar monk named Stynkfyst, who partnered with the largest random-pk guild of the time. Having been a former member of uber-guild Ascending Dawn, he had the knowledge the random pk guild needed to wake The Sleeper. The top guilds did not assemble their forces until word of Stynkfyst's intentions had spread, and it became clear that he intended to wake The Sleeper, forever preventing future guilds from farming the old loot table. Until this point, waking The Sleeper had not been seriously considered by any guilds, as it was believed that waking The Sleeper would make the offending guild's players kill-on-sight to the other guilds of the server. After 3 hours and 15 minutes, at 26% health, Kerafyrm disappeared (despawned). The players talked with the EverQuest Game Masters, and there was a general consensus that a bug had caused the problem, although some suggested (backed by statements from one GM) that higher-ups at SOE had purposely despawned Kerafyrm, because it was not intended to be part of the story.


The following day, the players logged in to find that Kerafyrm was back in his "sleeping" state, ready to be triggered again.


On November 17, 2003, after a nearly 3-hour battle, Kerafyrm was defeated."

He left no loot behind and disappeared 30 seconds after his death.

But...

"The first attempt that showed he could be killed was on a Taiwanese/Chinese server by the guild Peace of Formosa. (Formosa is an old name for Taiwan). They released a video showing Kerafyrm reaching 15% HP, at which point the warders respawned and they wiped. They conjectured that a GM intervened to stop them, as these dragons should not have respawned so quickly. Apparently they were not offered a second attempt like the later North American players were.

The next serious attempt on Sleeper was by the group of guilds on Rallos Zek. They got Kerafyrm to 26% HP when he despawned. They pointed fingers at GMs trying to maintain Sleeper being unkillable, or possible bugs. After players complained, SOE apologized and respawned Sleeper in his sleeping state.

The first Sleeper kill was by the guild Silent Oracle on the Taiwanese/Chinese server King of Thieves. The proof of this is a post in the thread linking to Chinese forums and a screenshot, and a thread about it on a Chinese forum. These posts are all before the Rallos Zek kill.

The second Sleeper kill was by the same group of guilds on Rallos Zek. After the GMs respawned Sleeper for them, they used the same tactics to finish the job. This one is widely considered to be the first Sleeper kill, but that is incorrect."
